Most organizational problems don't announce themselves as patterns. They show up as a string of individually reasonable events — a delayed approval, a reopened decision, an exception made for one client — and it's rare that anyone connects them until the cost has already added up.
This is a short, self-administered check for telling the two apart. It takes about a minute. It isn't a diagnosis — a pattern worth naming usually needs more evidence than a one-question check can hold. What it's built to do is simpler: tell you whether something you've noticed is worth a closer look, before it becomes expensive enough to force the issue.

Event vs. Pattern — a 60-second self-check
An event is a single miss: one late change order, one crew misunderstanding, one client complaint. A pattern is the same miss showing up in different clothes, three or more times, regardless of which project, which department, or which person was involved.
Ask yourself: can you name something that's happened at least three times, in different forms, without a single clear cause you could point to and fix? If yes, you're probably not looking at a person problem or a one-time mistake. You're looking at a pattern — and patterns are usually being reinforced by something structural: a workflow, an approval habit, an unwritten exception, a decision that quietly got made and re-made.
If you can name the pattern but not yet the structure reinforcing it, that's usually where the real cost is hiding — not in the last incident, but in whatever keeps recreating it.
